Extra Page

I created a kind of student progress report form. However, when I go to download it as a PDF there is an unexplained blank first page. I don't see it appear in other versions of the form. Is there something I have done or is this a technical error?

Thanks for contacting Jotform Support. It seems that there was a blank page on the PDF. You can delete it on the PDF Editor. Let me show you how:
