Olá, bom dia. Gostaria de saber se houve algum problema com a minha solicitação de VOUCHER das lentes zeiss. Fiz tudo o preenchimento como solicitado, e de acordo com as informações eu receberia um email com as seguintes informações: "Após o envio do formulário preenchido, em até 05 dias úteis, você recebera por e-mail seu VOUCHER cortesia". Porém, até agora não recebi nada.

Now, coming back to your question, keep in mind that Jotform is an online form builder. If you have filled out a form, you can directly reach out to the form owner so you can inquire regarding the voucher, since we are not directly associated with the form owner. You can reply to the email you received.
