We have got some problem with unique id last tuesday (20th)
Four numbers was twice
M000475 to M000478
But nobody changed anything in this form.
Can you explain this ?

Just looking at the three submissions from M000475, M000475 again, and M000474. They were all submitted one after each other on the same date from the same computer. Its possible that the system cached the unique ID.
I will escalated the issue to Level 2. The developers will take a look at this issue. And we will report back to you when we are ready.
