    My survey is online but no one can submit it due to :

    " Upload Error

    Files could not be uploaded for some reason.

    Please go back and fix the problem(s)""


    How can I fix this ?

    I tested your form, but I could not replicate the issue you reported. I successfully submitted the form and I was redirected to the Thank You page.

    Were you able to resolve the issue?
