Why did I get a notice to upgrade my account?

I just received a notice that I need to upgrade my plan. I have the bronze plan for 19. that is suppose to include up to 25 forms. I have only created 11 forms. I have included a screenshot of my forms.

What did I miss? Please advise!

I have checked your account with username: Huguette and it has silver plan which has 100 form limit.
