Smart PDF: Some of information does not appear in the form after I download

Hi. How to resolve when some of the information is not show in the form when i download the form. As you can see, the phone number is not appear.

Thanks for reaching out to Jotform Support. We are sorry that you are experiencing this issue. Rest assured that we are here to help you with it. It looks like this issue is caused due to the field not being mapped properly. You will need to open your Form Builder, then mapped the field as following:

You will also need to do the same to Area Code of the telephone field, and other fields you need to add into your PDF.
