How to limit users from using a unique code 3 times?

Ik wil een formulier maken dat alleen ingevuld kan worden als je een unieke persoonlijke code hebt ontvangen.

Er zijn ongeveer 2000 van deze unieke codes beschikbaar en elke code kan maximaal 3 keer worden gebruikt.

Hoe kan ik dit toepassen in Jotform?

Now, coming back to your question, I'm afraid that there is no option to limit your form responder from using a unique code 3 times. However, we have the Unique Field feature that restricts your form responders from using the same code more than once.

I don't want random codes to be able to be entered.
How can I link my generated codes? So that only the codes distributed by us allow access to the form?

Thanks for getting back to us. If you want to set up a unique code, you may want to try the Spreadsheet to form widget. This will allow you to upload a database using an Excel file, which will automatically prefill the form fields depending on the code entered. See screencast below:

You can test this Demo Form and see if it works for you. Here's a guide you can follow on How to Use the Spreadsheet to Form Widget. Then, you can set up the Unique Field to limit the unique code entered.

How can I apply the unique field to the unique code from the Spreadsheet to Form Widget? I cannot select this field in the Unique Field settings.

We really appreciate your patience and understanding while we’re looking into this. Unfortunately, it is not possible to apply the unique field to the unique code from the Spreadsheet to Form widget. You can still use the Spreadsheet to Form widget to use your own unique codes, but as for limiting the submission once, you can follow the suggestion of my colleague above using the Unique Field feature, since using 1 code in 3 different submissions, is not possible as well.
