My form was disabled as I had gone over the limits of my account. I have re-enabled the account by joining the Bronze Plan. It's a PayPal form and I wondered if I needed to renew any information, or will the form work just as before it was disabled without any additional intervention?

Once you upgrade your account your forms will start working without need for you to add additional information.
I can see your forms are working ok.
