How to change the image preview of the form when sharing in WhatsApp?

Thanks for reaching out to Jotform Support. The Jotform logo is shown when there is no logo present in the Header of the form. We use it as a default preview image. I've checked your form, but there was no logo added, please try adding one. It's easy, let me show you how:

1. In Jotform Builder, click on the +Add Your Logo and upload your logo.

Once added and your logo is still not showing, it is possible that the browser cached the default thumbnail so it would be great to give it a few tries by refreshing WhatsApp or removing the 'form' part from the URL using it as https://www.jotform.com/form/240360999395874. Also, WhatsApp tries to retrieve the preview image when you paste the form link. So, you can wait a few seconds until seeing the preview image before sending the message. Take a look at the screenshot below to see my results:
