    Hello fbking.  Am I correct to assume that the form does not apper when you use SSL on your website?  If so, it is because you have embedded your form not using SSL.  It is normal for any browser to block contents of an SSL based website that are coming from non-SSL based sources.

    Example: On your visiting a website using HTTPS but inside it are only using HTTP.

    The visitor can still see the contents of the site provided that on the prompt, they will allow downloading contents coming from non-secure sources.

    SOLUTION:

    1) The fastest solution would be just use or enable the form to use SSL by default.  You can enable your form's SSL by following this user guide.

    2) If you can code your webpage using PHP, you can try this:

    <?
    if (
    $_SERVER['HTTPS'] != "on") {

    ?>
     <script src="https://secure.jotform.us/jsform/22035134349144" type="text/javascript"></script>

    <?
    } else {
    ?>
     <script src="//form.jotform.us/jsform/22035134349144" type="text/javascript"></script>

    <?
    }
    ?>

    3) If you cannot do PHP but can insert javascripts, you can try this:

    <script>
     var jotsourcesecure = '\<\script src="https://secure.jotform.us/jsform/22035134349144" type="text/javascript"\>\<\/script\>';
     var jotsource = '\<\script src="//form.jotform.us/jsform/22035134349144" type="text/javascript"\>\<\/script\>';
        if(top.location.protocol=="http:")
        {
          document.write(jotsource);
        } else {
          document.write(jotsourcesecure);
        }

    </script>
