Your survey with the notes doesn't work

    It freezes up.  It wouldn't let our tester ever complete the form.  We are switching away.  I just wanted to let you know.  We also had good results with the old jotform, but just can't take the chance of having problems.

    I checked your form and I can verify that the settings are configured correctly. 

    I did a test submission in your form ( and I was able to submit it. It did not freeze up. I am using Google Chrome on Windows 10 PC.


    Please take note that your Thank You Page is set to redirect to "".

    Is this form embedded on a webpage? If yes, then please share the URL of the webpage so that we can further investigate.
