Why hidden fields are showing?

The top to questions ("Are you licensed..." and "Did you earn..." with the drop down fields) as well as the "Suffix and/or Degree" field are hidden in the form, but will not go away on the live version of the form. I do not want to have to delete these fields.

Can you please assist?

You have conditions 2, 3 and 7 that are involved with those fields:

So if any of those conditions aren't true, those fields will be shown.

So you have to modify conditions if you want that field hidden all the time.
