How to change Page title?

I have created a form by cloning another one. When I share the form by SMS, it is coming up with the original name (MBSR registration) instead of the edited name which should be TRTP intake. I’ve looked for all possible places I can edit and can’t find anywhere else.

Thank you for reaching out to Jotform Support. I checked the Form settings, and the Page title remains as MBSR Course Registration. To change the page title, please go to the Form Settings of the Form builder settings. Please refer to the screenshot below:
