    Hi Eng:Aws,

Thanks for reaching out to Jotform Support. We apologize for the inconvenience this suspension caused you. Please note that we have an Asking for username and password is not allowed in Jotform. After checking the form, I was able to confirm that you are only using it to lock the form. As a workaround, you can restrict who can fill out your form. We’ll just need to enable Password Protection for that. This means that people you send the form link to can only access your form after they enter the password that you share with them. Setting it up is easy. Let me show you how:

    1. In Form Builder, click on Settings in the orange navigation bar at the top of the page and click on Show More Options.
    2. Next, scroll down to Password Protection, and put a check in the box next to it.
    3. Now, enter the password that you want to use and that’s it. You’re done.

Now, anyone who wants to access your form will have to enter the password you gave them:

I have gone ahead and activated your account again. Your form is back online and should work as expected. Do try to sign in to your Jotform account and get back to us if you have any trouble.
