Need help getting into account to get my cats avid chip number

I got my cat nuetered and an avid microchip put in him but when I hit the pdf file that takes me to ur jotfrom page. It tells me oops and his page isn’t available we can’t find ur account…. I was just trying to find his microchip number so I can set it up can someone please help me
