Why is my lightbox form displaying blank?

    Hi. I have added a lightbox form to a page on my Wordpress site. When I initially click a "register" button, the lightbox pop up comes up, but the form is entirely blank (see attached). Once I click somewhere, the fields appear. Do you know why this would be happening?

    Hi,

    In your site there is a Javascript Error.

    This error can cause an error on initializing lightbox correctly. If you have a change to correct this JS errors, probably your lightbox going to work properly.
