Evaluation Form Survey

Hello,

I created an evaluation form survey using the cool, new interface. Thank you for these awesome templates!

I added an Autorize.net widget to the end of the survey for an optional donation. Even though I unchecked required in the Settings, the user is still unable to submit without making a donation.

Will you please help me create a truly optional donation at the end of my survey?

If you turn off the option Suggest an Amount you should be able to submit the form without filling the credit card information.

When you set a Suggested Amount then the payment information is necessary for that suggested amount.
