How to lift suspension on the account

so i need to create a form to help people 

i need to credit a form to help people with login in for there software but you do allow that so what is it that you would like me to do

Thanks for reaching out to Jotform Support. I checked your account and it's currently suspended because your form Attack Assistant Program have fields asking for login credentials like the Username and Password. Please note that this is not allowed according to Jotforms Terms of use. You can still use the form but the offending fields needs to be deleted.Since there's no submission received on the form yet, I have deleted the offending fields. I have also cleared your form cache and lifted the suspension on the account. Please try logging out and logging back in.
