I am trying to login to jotForm, but it tells me I have been suspended can you tell me why this is? I did not receive an email or warning.
What is the process to resolve this?

Upon review of your account it was found form:
http://www.jotform.us/form/80600582235147 to be collecting Driver's Licenses and the header contained copyright logos of insurance companies that are not reflected in your company's domain.
In order to be able to collect driver's licenses it will only be allowed the collection of it's numbers with the mandatory signature and agreement widget:
2. Add a Signature widget where your users sign and agree to provide such sensitive information.
We deleted the guilty fields in order for you to be able to work on them . First you have to add the widgets mentioned above and then request the desired information.
We have reactivated your account and made your form available in order for you to continue working with it.
