Trying to find certain form

A user wanted me to take a look at this form https://form.jotform.com/232534657275058 to see who is getting the emails for it, but I can't find it on our account. Is it possible to look it up for me?

Thank you, Rose. Typically the forms we use come from this account, which is linked to our BizApps email, so maybe this form was created on a different account with an H email, and that's why I can't find it on the BizApps account.

The first email is our ARCO Murray charity email, but I'm not sure who the second email with the H would be. Would you be able to give me the whole thing so I could run it by the user that wanted me to take a look at this form? If not, I can just send her H******@arc***r**y.com and see if she has any ideas.
