I'm using a custom rating scale (from very good to very bad instead of very bad to very good). With css I've made the numbers invisible but when I hover over the radio button the rating number still appears in a hover-box. This is confusing for the people who have to submit their form, because 'very good' corresponts with '1'. How can I disable the automatic hover?

I dont think this is going to be possible it is default behaviour of the browser to render the content of the 'title' tag when hovering over an element that contains it.
The only way I can see you rectifying it, is by downloading the forms source code and editing the 'title' tags used for these elements directly.
You can change the those title="1", title="2" etc to title=""
