    I am a chiropractor and I am trying to create a form to collect new patient insurance information. Patients often send pictures of their insurance cards via email and Jotform will provide a more secure way for patients to submit their information to our office.

    As soon as I created a question titles "Member ID" I got locked out of my account. Can I not collect insurance info via Jotform?

    Upon reviewing your account we found the form :

    http://www.jotform.us/form/80727523584159 to be collecting Member ID. It triggered the autosuspension.

    What we would suggest is to add a signature and an agreements widget:


    1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities


    2. Add a Signature widget where your users sign and agree to provide such sensitive information.


    In that way we can assure you the forms won't draw unwanted attention.

    Please add the widgets first and then request the desired information in order for the system to recognize a valid collection.

    No upload buttons are allowed for digital copies, nor the widget will secure an upload button. You can use text fields with the widgets.


    Your account is active and your form available.
