Form fields not showing in the live form

I am designing a SURVEY form. Initial stages, It is showing question # 8 inspite of displaying Q# 1.

Please guide how to add survey condition for continue or termination. Look at the form.

I checked your conditions, and it seems that you are trying to show and hide form fields conditionally; however, please note that all your form fields are showing by default, they are not hidden, but you have used them in your show condition. You will need to hide the fields to be able to show them conditionally. For example, your last page:

This page is already showing by default, but you have used them in your show condition.
