Please provide feedback on why my account has been suspended and what I need to fix.

Thanks for reaching out to Jotform Support. When I checked the account associated with your email address, I saw that it was suspended due to this form in the account collecting login credentials:
This is against our Terms of Use. Therefore, I’ve removed the offending field on your form and lifted the suspension on your account. Please refrain from making new forms with fields that are against our Terms of Use to avoid this kind of thing from happening again.
