How to present a clear form for publishing.

    Hi,

    When people go to the published form it's already filled with the information I used for the test run. Did I click something I shouldn't have? Please advise how to present a clear form for publishing.

    I checked the page URL you provided and it is a clear form on my end. I also check your Form Settings and the Autofill is disabled.
