Submission page: missing fields and the submit button

I'm trying to edit the submissions on my form, but it is not showing me all of the fields or the submit button.

When I checked your form, you have conditions that would hide the submit button.

The only single choice that would not show the submit button is the "Requesting a Tax Exempt Certificate".

Cloned your form and edited my submission, and I can see all the fields and the submit button.
