Spreadsheet to Form: Why My Form Keep Jumping?

Every time I enter a member code in a form that I have, the form keeps jumping. This was working perfectly up until a few minutes ago it started. Your assistance will be greatly appreciated. Form https://form.jotform.com/241128289589167

Thanks for reaching out to Jotform Support. I cloned your form and tested it to see if I could replicate the issue, and I ran into the same problem:

To fix this issue please disable the mentioned condition below and if possible change the theme. I can see that you are using the old-form theme which could be the reason for the issue. Check out the steps below to see how to change the form and use the latest theme:

• Open your form in Form Builder and click on the blue Paint Roller icon on the right side of the screen.
• Under the Themes tab, select the theme "Default theme".

Here's the cloned version of the form that you can test. Please check the cloned version of the form and make sure everything is working as you like before changing the theme. If you want to continue with the current theme you can try disabling the below condition only:
