Our url for our apply now jotform application is listed below.

    http://valleymedicalstaffing.com/apply_online.php

There are fields showing titles but no widget appears below. Also fields are missing functionality.  WHile in Build the form looks perfect and it also looks good on the preview but on the active website fields are not functioning or showing complete data for input.

It looks like there is a script conflict.

Can you please try embedding the form using the Iframe code?

You can find the iframe code for your form herehttps://pastebin.com/raw/SQsJJG7n

You could replace the current code in your website.

Your form is embedded in table:

There you can just replace the code with iFrame code.

If you need any assistance please let us know.

    Thank you!

HI, Is there an easier solution. I am not a programmer and have no clue to know how to find the HTML iframe and then know where to paste the embed code.

I do not understand why I can't simply delete the widget in builder and replace it and then publish?

I was able to reproduce the problem. There are errors when the form is embedded using its script embed code. This is probably the reason why the widgets don't show up. Here's my test: https://shots.jotform.com/elton/thread1417591.html

I will forward this to our developers so they can further investigate. We will let you know once this is fixed.

For now, please consider my colleague's solution. That is to replace the existing embed code on your website with the iframe embed code.

The issue should be fixed. Please kindly re-check the form.
