We have a form set up and set it to send notifications to two email addresses. It was working fine for a period of time. A few weeks ago, only one email address was receiving notifications. I didn't change anything, so I'm not sure what is going on?

I have checked the email notification set up in the form that is embedded on the website you shared and I only saw 1 email recipient.
I also checked the email history of your account and the logs don't show any email alerts sent to multiple recipients at once. — Send-Notifications-to-Multiple-Recipients
If you want to include another recipient in the email notification in your form, you first need to upgrade your account. The Multiple Recipients feature is only available to paid users.
