Need Help with JotForm Integration

Hey folks,

I'm having trouble getting Jotform to work on a client's website. Tried a bunch of stuff, but it's just not cooperating. Anybody else been through this? What did you do to fix it? I've checked the code and stuff to find more, but I'm stumped. Any advice? Thanks!
