    Hello positiveplans.  It appears that Paypal forced your subscription to be canceled due to a missing username.  I recall that you have had it changed a month ago. 

    Since it's also already time for your subscription, I recommend that you simply resubscribe today by following this link: http://sites.fastspring.com/interlogy/product/jotformpremium?referrer=positiveplans

    In addition, please do login to your Paypal account and check your most recent subscriptions to make sure that the existing subscription has already been canceled before proceeding with re-upgrading.
