I received the email notification when I submitted the form I created but I can't find the form in my account

    I was able to locate the form with the help of the information you showed us in your screenshot.

    The form belongs to a guest account (not registered account). We normally transfer forms from a guest account to our user's registered account. However, in your case, the guest account is in a suspension status because the form you are looking for is being flagged for phishing. The form has fields that collect bank details, like credit card number.

    I would like to apologize for any inconvenience. Please know that we have an anti-phishing system that automatically put accounts into suspension when found that it contains a form that asks for information which is non-compliant with our terms.

    I actually just did a test to see if it actually works. I just entered any
    details on the form. Cant the info be deleted and forwarded to me. I just
    need the form format because I have spent allot of time on it creating it.

    You can clone the form and modify if you want. In case your account gets suspended, please reach out to us and we will delete the field(s) that will be flagged by our anti-phishing program.

    We have now deleted the guilty fields on the form http://www.myjotform.com/form/80753747857573 and moved it to your account now. However, I notice that the form http://www.myjotform.com/form/80774074301553 is also collecting some sensitive information that could cause the account to be suspended. Please refer to our terms of usage for more information on collecting sensitive information. Please remove the guilty fields on the form to avoid the suspension on your account.

    Hi there I created a new form and my account has now been suspended. I did
    submit an explanation why I need the form. Could you kindly give me
