    9) There are 2 boxes below to enter your code you got from JotForm. The first box views your Facebook form for everybody, and the second one is for fan views only. Paste your code as you wish and save.

    The boxes are actually the Text boxes down below "Enter your Content here", these boxes are shown for either everybody view (it is not required to be a fan) or fan view (only fans will see the page)

    My colleague is right! but unfortunately as of the moment Static HTML app is still down due to a problem with their amazon servers so I can't provide you a screen.
