Can you please share with us the FormID or the URL of your page where the form is embedded? as we would like to investigate further

The links are working fine.
I have looked into your account and I see that your forms have been dissabled.
You can simply log into your accoutn and enable them.

I have found a ticket for a similar issue triggered when a guest account is migrated to a registered account. Also, a change of user name can trigger this bug. Are any of these actions things that you have recently done?
If so, then I will add your case to the ticket and have our dev team look at it as well.
If not, I can forward this as a new bug.
I have cloned one of your forms and tested the clone without being able to reproduce the issue.
That may be a workaround for the time being. Please give it a try. Clone one of your forms and test to see if the issue persists on the clone.

We have not changed the user name, but have upgraded to a paid account several months ago. We're experiecing the same problem from some submissions on this form as well: http://www.jotform.com/submissions/22336190014139?
Is there a workaround so we can access the uploaded files?

I think there is an intermittent issue as I was not able to access to the file Jonathan was, but I am able to open the first one you posted on this thread .
So I have forwarded this issue to our next tier for further investigation
