Hi RH_Ministries,
Thanks for reaching out to Jotform Support. You can add more elements and match them to your PDF document. Let me show you how:
- In Smart PDF Forms Builder, click the Add Form Element option on the left side.
- Under the Basic tab, drag and drop the element to your form. If the field is multiple-choice, you can add a Multiple Choice element. But if it's single select, you can add a Single Choice element.
- Then label the field and add an option.
- Next, map the position of the fields to your PDF document and map it.
Then do the same steps above for the other questions that did not automatically add a field. We also have this guide to learn more about adding an element and mapping the fields.
