Would you be able to help me create a form?

I have been using Jot Form and I think it is great.  Would someone because to help me create this form in a Jot Form?  http://www.dbsalliance.org/pdfs/MDQ.pdf

I think that's possible to do, you will basically need to some radio buttons (Single Choice) and you may also use some text elements to display the needed information that does not require an input.
