Suspended account

I am giving out yard signs in my area and needed help collecting and keeping information straight. Thought this was that type of platform. When I uploaded my campaign image it immediately suspended account. I run a group called Just More Than Survivors of Suicide in Kansas City and the Missouri 988 Crisis Lifeline is sending me 100 signs to hand out. I hope this isn't restricted because of the suicide stigma. Please let me know or just delete the account. Also, I am already 'logged in' and everytime I try to make it private it makes me log back in. This is an endless loop and not very helpful. Thanks.

Thanks for reaching out to Jotform Support. Your account has been suspended because of your Sign Up Form, which was collecting login credentials. Collecting highly sensitive information such as login credentials/passwords is against our Terms of Use. I've now deleted the offending fields and reactivate your account, so try to log in back and check if you'll still see any error message.

I didn't ask for it to put that stuff in those forms. It automatically populated and I didn't like the form so I thought that I deleted it and started my own blank form without those questions. I may take a look and try again we'll see! Thanks for the help.
