Images not displaying on the form embedded on Wordpress

Hi I am trying to embed a form on a Wordpress site ( http://dev-sammitr.3pm.digital/canopies/configurator/ ) but the images keep saying 'loading . . .' without actually loading...

The widget I am using is called 'Image Picker'
