How to lift suspension on the account

    I am not sure why it was suspended. Please help. The form I created was a legitimate business form. Please let me know how I can get this fixed. Thank you!

    Hi Shondi,

    Thanks for reaching out to Jotform Support. Your account has been suspended as the following forms have fields asking for the credit card information which is not allowed according to Terms of use.

    • Credit Card Authorization Form
    • Credit Card Authorization

    Before we can reactivate the account we would need to delete the offending fields. Please get back to us with your confirmation.

    As soon as we hear back from you, we can move forward with a solution.

    Thanks for getting back to us. We have deleted the offending fields and activated your account. You should be able to access your account now.
