I had made up a template and have used it a few times no problem. It seems now I am unable to access it. Can you help me retrieve it?

Upon checking the form you shared, I have noticed that it belongs to the quest account, which is in DORMANT status at the moment. It means that you have might created it before you signed up. I have now reactivated that account, so please clone the form to your main account not to loose it once again.
