IP 110.139.161.196 Banned

hello jotform support.

I experienced IP information banned.

Even though I am a regular user, with normal activities.

I am not a phisher.

Thanks for reaching out to Jotform Support. I checked your account and noticed that the ban is due to the form that you have created contains fields that collect login information such as a combination of email address and password. The collection of such information is against Jotform's terms of use and this is why your account got suspended.

We can re-enable your account as long as we remove the fields that are flagging your form. So we would like to get confirmation from you that you are allowing us to remove these fields and once we get the authorization from you we'll work on this and reactivate your account.

As soon as we hear back from you we'll help you more with this.
