Product List: Product options are not showing

In the Form review screen below, the options are not displaying as per setup in Form Building shown above. Please advise CSS Code to resolve.

Thanks for reaching out to us for help. As seen in your screenshot, you don't have the product itself selected. You must select the product for the options to appear. Check this out:
