Form is disable due to HIPAA

This is really causing a problem for me. These forms do not contain hippa information.

Thanks for reaching out to Jotform Support. I assumed your referring to your W4 Employee’s Withholding Certificate form. I have checked the form, and it seems that the form is active.

Are you perhaps referring to a different form. Can you share the form link so that we can check it out and investigate further?

Thanks for getting back to us. Can you share with us the form URL so we can investigate it? You can find the URL of the form in Form Builder. Here's how:

• In Form Builder, go to the Publish tab.
• Under Quick Share, click the Copy Link button, then share the copied URL with us.

These are the URLS that are grayed out. I need this fixed as soon as possible please. We are paying for a service and it is not working properly.

Thanks for getting back to us. It appears that your forms are no longer HIPAA-locked. You'll just need to enable the forms by clicking on the More button and clicking Enable. Check out the steps and screencast below to see how it's done:

You can also set the status of your form to Enabled in Form Builder. Let me show you how:

1. Open your form in Form Builder.
2. In the orange navigation bar at the top of the page, click on Settings.
3. In the Form Settings window, set the Form Status to Enabled. That's it, you're done.
