Give access to each teacher to fill out a form

    I would like to set up a form for our schools behavior plan. Each teacher would need access to the form to fill out each day for multiple students behavior throughout the day. I would like this for to be submitting to an larger table that would be easy to access for our admin. Is this possible? Can we talk about how this could happen?

    Hi Brandi, 

    Thanks for reaching out to Jotform Support. You can create your form or select from our Education templates. Then the teachers can submit the form for their students' behavior. All the submissions can be managed by Jotform Tables in one place. I noticed that you already have a Level 1 Behavior Form for the same purpose. I cloned it to show you how can we add access to the teacher:

    Add two Short Text elements for the Teacher's name and their passcode:

    1. In Form Builder, click on the Add Form Element menu on the left side of the page.
    2. Under the Basic tab, add a Short Text element to your form.

    Now, under the same Basic tab, add a Page Break element:

    Now, you can add a condition to show the second page if the entered teacher name and access code are correct:

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
    2. Click on Conditions on the left of the page.
    3. Now, click on Add a New Condition and then click on Show/Hide Field.
    4. Set the conditions as you see below:

    Now, we can check the result after applying the condition:

    Thank you for this information this is amazing! I was however looking for more information on how the form shows up in the tables afterwards.

    1. Can more than one administer have access to the table
    2. Can I sort the forms by each teacher
    3. Can I sort the forms by each student

    Hi Brandi,

    Thanks for reaching us back. You can use the Share option in Tables, so other users can have access to the submissions. Doing that is easy. Let me show you how:

    1. In Tables, click the Share button in the upper right corner of the screen.
    2. By default, Tables are set to Private. You can click on that or the gear (Share Settings) icon to modify it.

    3. By keeping the Tables Private, only invitees can have access to it. Change the Role depending on your needs. You can also Apply Limitations for other users to utilize.

    4. Once done, you can share the link provided in the settings by clicking Copy Link and send it to any available medium on your end (e.g., email, instant messaging, etc.).
    5. You can also enter the email addresses of the users via the Invite By Email option.

    6. The invited users will then receive an email to access the Tables.

    As for sorting, I believe you are pertaining to sorting submissions by field entries. It's easy to do. Let's start:

    • In Tables, click the dropdown icon (v) for a field and choose either Ascending or Descending from the list shown in the blue box.

    Please note that you can only pick one field to sort within the Table. You can just switch fields if you need to sort a specific column based on your needs.
