Multiple Blank popup form issue

Hi,

I am using lightbox popup javascript,  when I am using close button for hiding popup form and again click the button than 2nd popup is coming, oneblank form is coming  with 2nd correct popup form.

Please help me how to remove blank popup form, I want only my correct jotform.
