Hi Jum Gucci,
Thanks for patiently waiting. Yes, you can embed the same form on another website, which will include all of your hidden fields as well. Let me walk you through the steps below:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- In the menu on the left side of the page, click on Embed.
- Now, click on the embed option you want to use, and copy the code. For the demo, I've used the I-Frame. That's it
- Embed your link on your webpage, and you're done.

I think you misunderstood my question. I already have a form embedded on multiple URLs (by simply copying and pasting the embed code). What I'd like, if possible, is to know which one of those URLs a visitor was on when he/she submitted their entries. Is that possible? Or, do I need to have unique forms for every URL (in order to identify them)?

Hi Jim,
Thanks for getting back to us. I’m sorry that this is happening. You have the option to incorporate the Get Form Page URL widget, which detects the webpage where the form is embedded and provides the page URL each time the form is submitted. Let me show you how:
- In Form Builder page, select Build located at the top orange menu bar.
- Choose Get Form Page URL under the Widgets tab and add into your form.
- Now, let's ensure it's hidden from your form fillers by clicking on Properties(Gear Icon).
- Enable the Hidden Fields under the Advance tab.
