My account is suspended

    I'm not sure why my account is suspended.

    The suspension was caused by our anti-phishing filter. Your account has been re-activated now.

    Since you are requesting sensitive information on the form, it is recommended to update the JotForm account email address to the official organization email address.

    My account is still in suspension status. How do i reactivate it?

    This time the account was suspended due to requesting usernames and passwords through forms.  This is against our terms of use:

    If you would like, we can remove the username and password field from the form and activated the account.
