Remove suspension

I use Jotform to collect information for applications and loans/deals for my buisiness.

Hi Daniel,

Thank you for reaching out to Jotform Support. The account is suspended due to phishing activity. The form is collecting sensitive information, which is a violation of our terms of services. In order to reactivate the account, we must delete the offending form.

If you feel that you are a legal institution and that the account should not be suspended, please provide us with the information below:

• The URLs where your forms are embedded and being used.
• Any documents that will prove you are running a legit business.
• Proof of affiliation to any legitimate institutions or your website.

Send this proof to reviewers@jotform.com with THREAD (ticket #) as your subject. We will let you know about the status of your account after a thorough review of it. We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.
