Account Suspended for Fake Service

My account has been suspended. My gotm doesn't go against you all policy.

Please advise.

Thank you for reaching out to Jotform Support. As I checked, your account has been suspended as your Auto Sales Application Form and Car Loan Application Form have been flagged as a Fake Service. We need to delete it before we can reactivate your account. If, however, you are a legitimate institution and don't want us to delete the flagged form, then we need you to provide the following information.

• The URL where their form is used.
• Any document that will support your claims.
• Your website or proof of affiliation to legitimate institutions.

Please send these proofs to reviewers@jotform.com with Thread #14242561 as your subject. We will let you know about the status of your account after a thorough review of your proofs. We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

I have no idea where those forms have came from. My form is for my travel agency.
