Why are the fields not appearing in email?

I have set up form https://form.jotform.co/80761655749873 to have specific fields where mail notification is sent. But when I am receiving emails the fields are not appearing. See images.

I presume this is resolved? I found out that it was just a number issue on the field names which I can see that you've already corrected. That last 2 fields should be:

{contact[4]}

{contact[5]}
